We are the organisation responsible for the personal information described in this policy.
02Information we collect
Information you give us. When you use an appointment request form, an enquiry form or a newsletter subscription form on the Site, you may give us:
- your name;
- your telephone number;
- your email address;
- your postal address;
- the service, department or treatment you are interested in;
- your preferred appointment date and time;
- any note or message you choose to write in the form.
Please share only what is needed. Do not use the form’s message field to send detailed medical history, reports or test results; we will collect clinical information securely when you attend the hospital or through our clinical team.
Information collected automatically. Like most websites, our hosting and the third-party services described in section 6 may automatically record technical information such as your IP address, browser and device type, the pages you visit and the time of your visit.
Hospital and clinical records. If you receive treatment at Shrey Hospital, your medical records are created and maintained separately, as confidential health records, in accordance with applicable law and our internal hospital policies. Those records are not collected through, and are not governed by, this website policy.
03How we use your information
We use the information collected through the Site to:
- respond to your appointment request or enquiry, and contact you to confirm, reschedule or answer questions;
- arrange the consultation, department visit or service you have asked for;
- send you the information you have requested about our services, and email you copies of your own submission where the form is configured to do so;
- maintain internal records and improve how the Site and our services work;
- meet legal, accounting, billing and regulatory obligations; and
- keep the Site secure and prevent misuse of the forms.
We do not use the information submitted through the Site to make automated decisions about you.
04Consent
Each appointment and enquiry form on the Site includes a consent checkbox. By checking it and submitting the form, you confirm that the information you have given is accurate, and you consent to us contacting you and using your information for the purposes described in this policy.
You may withdraw your consent at any time by writing to info@shreyhospital.in. Withdrawing consent will not affect any processing that already took place, or the handling of records we are required to keep by law. If you withdraw consent while you have an appointment or treatment in progress, we may need to contact you by telephone to close the matter out.
05Cookies and similar technologies
Cookies are small files that a website stores on your device. The Site uses cookies and similar technologies to keep the Site working correctly and to understand how visitors use it. We do not currently use cookies to display advertising to you on other websites.
You can block or delete cookies at any time through your browser settings. Blocking them will not stop you from reading the Site, but it may affect parts of it.
06Third-party services used on this Site
Some parts of the Site are provided by third parties. When a page that contains them loads, those third parties may receive your IP address and may set their own cookies. They operate under their own privacy policies, which we do not control:
- Meta / Facebook Pixel — used to measure the performance of our Facebook and Instagram pages and campaigns. It loads when a page of the Site loads and reports page views to Meta.
- Google Maps — an embedded map that shows our location, loaded on the Contact Us, About Us and similar pages.
- Google Fonts — used to display the Site’s typefaces.
- YouTube — used to embed video content on some service pages.
- WhatsApp — a click-to-chat button that opens WhatsApp when you choose to use it.
Our website hosting and email are provided by third-party service providers who process data on our instructions.
07When we share information
We do not sell your personal information, and we do not rent or trade it. We share it only in these situations:
- Our clinical and hospital team — doctors, nurses, technicians and administrative staff who need it to arrange your visit or provide your care;
- Diagnostic and service partners — laboratories, imaging and other service providers involved in your investigation or treatment;
- Insurers, TPAs and employers — only where you ask us to, or where you are using a health scheme or insurance policy for your treatment;
- Technology providers — hosting, email and IT providers who support the Site and our communication systems, under confidentiality obligations;
- Legal and regulatory bodies — where we are required to disclose information by law, court order or a lawful request from a public authority; and
- Emergency situations — where sharing is necessary to protect your health or the health of another person.
08How long we keep information
We keep information submitted through the Site only for as long as it is needed for the purpose it was collected for, and then for as long as we are required or advised to keep it — including to respond to your query, to keep our accounting and billing records, and to satisfy legal, regulatory or clinical record-keeping requirements.
Enquiries that do not lead to an appointment are kept for a limited period and then deleted or archived. Clinical records are retained for the periods required by applicable medical and hospital regulations.
09Security
We take reasonable technical and organisational measures to protect the information we hold, including restricting access to it to staff who need it, and using secure connections (HTTPS) on the Site. Please note that no method of transmission over the internet, and no method of electronic storage, can be guaranteed to be completely secure. Please avoid sending sensitive clinical information through the website forms.
10Your rights and choices
Subject to applicable law, you may ask us to:
- confirm what personal information we hold about you and give you a copy of it;
- correct information that is inaccurate, incomplete or out of date;
- delete information that we no longer need to keep;
- withdraw the consent you gave through a form on the Site; and
- stop sending you non-urgent communications.

We may ask you to verify your identity before we act on it, and in some cases we may not be able to comply — for example, where the law requires us to keep a record.

11Children
The Site is meant to be used by adults. Appointment requests for a child should be made by a parent or legal guardian, who is responsible for the accuracy of the information submitted. We do not knowingly collect personal information from children through the Site without the involvement of a parent or guardian.
12Where your information is stored
Our website and email services may use servers and service providers located in India or in other countries. Where information is processed outside your location, we take steps to ensure it continues to receive an appropriate level of protection consistent with this policy and applicable law.
13Changes to this policy
